Most Weight Regained by Older Women Is Fat, Study Finds (HealthDay)

THURSDAY, Dec. 22 (HealthDay News) -- Some weight regain is common after weight loss, but in older women many of those regained pounds return as fat mass rather than muscle mass, according to a new study.

How this affects strength and health needs further study, but experts said the findings underscore the downside of so-called yo-yo dieting.

The study included 78 women, average age 58, who had lost about 25 pounds during a previous diet study. Looking at data on the regainers, the researchers found a change in body composition.

"A third of the weight lost was muscle," said Dr. Barbara Nicklas, a gerontologist at the J. Paul Sticht Center on Aging and Rehabilitation at Wake Forest Baptist Medical Center in Winston-Salem, N.C.

"The proportion of weight they gained [back] as muscle was 20 percent." That left them with more fat and less muscle.

The researchers evaluated the women's body weight, lean mass and fat mass before the diet, right after weight loss, and six and 12 months later. At one year, the researchers zeroed in on 68 women for whom complete records were available. Fifty-two (76 percent) had regained some weight, including 11 who weighed more than at the study's start. Sixteen were still losing weight.

The study was published Dec. 13 in the American Journal of Clinical Nutrition.

The women in the study were sedentary, and their average body mass index (BMI) at the start was 33. BMI is a calculation based on height and weight, and a BMI of 30 is considered obese.

The finding may not apply to women who are less overweight, Nicklas said. She suspects the results would also apply to older men, but perhaps not to younger men or women.

"Some research shows [younger people] regain their weight in the same muscle-fat proportion they lost [it]," she said.

There is a possibility, Nicklas said, that a higher proportion of fat with the regained weight may simply reflect normal aging.

The study of regain after weight loss is a relatively new area, said Dr. Alice Lichtenstein, a professor of nutrition science at Tufts University, Boston, who is familiar with the study.

"There was a wide range of variability in both weight loss and regain among the postmenopausal women studied," she said. The shift in body composition toward more fat than muscle didn't occur in all women, she said.

Even so, she said the clear message is to try to avoid becoming overweight to begin with, and to keep off the excess weight once you lose it.

"For those women who have gained excess body weight and are then successful in losing it, this finding may add a bit more impetus to maintain the weight loss," Lichtenstein said.

But the study shouldn't deter overweight individuals from trying to slim down, she said.

Protein consumption is important to help minimize the amount of muscle loss, Nicklas said.

U.S. dietary guidelines recommend 0.8 grams of protein (or more) for every 2.2 pounds of body weight. For instance, a woman who weighs 200 pounds should take in about 70 to 90 grams of protein, Nicklas said. A 5.3-ounce container of Greek yogurt has about 13 grams or more of protein, she noted.

Exercise and a balanced diet can help maintain weight loss, Nicklas advised.

9-year-old NYC boy chokes in school cafeteria (AP)

NEW YORK ? Family members and a witness say a New York City fourth grader choked on meatballs during lunch while school cafeteria workers stood by. The boy later died.

The New York Post ( http://nyp.st/td2pmw) reported Saturday that 9-year-old Jonathan Jewth fell to the ground during lunch Dec. 5 at Public School 47 in the Bronx. He was unconscious before help arrived.

A parent at the school, Andrea Perez, told the newspaper she saw the boy choking but cafeteria workers did nothing. She said at one point they yelled at him to put his fingers down his throat. Jonathan's family says he suffered brain damage and died Monday.

Schools Chancellor Dennis Walcott says his death was a tragedy. Education officials haven't responded to requests for comment about the family's and Perez's account.

Senate OKs short-term extension of payroll tax cut (AP)

WASHINGTON ? Senators racing for the exits after a year of bitter battles passed legislation Saturday that would extend a Social Security payroll tax cut and jobless benefits for just two months, setting the stage for the next fight until February.

While a partial victory for President Barack Obama's year-end jobs agenda, the measure awaiting House approval next week contains a provision demanded by Republicans to pressure the White House into approving construction of a Canada-to-Texas oil pipeline that promises thousands of jobs.

Democratic and GOP leaders option for the short-term extension after failing to agree on big enough spending cuts to pay for a full-year renewal of the payroll tax cut. The 2 percentage point tax cut affects 160 million taxpayers. The weekly jobless payments average about $300 for millions of people who have been out of work for six months or more.

The measure was approved by an 89-10 vote during a Saturday session.

Votes were scheduled later Saturday on a $1 trillion-plus catchall spending measure setting the day-to-day budgets of 10 Cabinet agencies. The House cleared the spending bill Friday.

In a statement, White House communications director Dan Pfeiffer indicated Obama would sign the two-month extension measure, saying it had met his test of "preventing a tax increase on 160 million hardworking Americans" and avoiding damage to the economy recovery.

The statement made no mention of the pipeline.

The legislation, would require the president to grant a permit, but allows Obama to opt not to do so if he determines that the pipeline is "not in the national interest." One senior administration official said the president would almost certainly refuse to grant a permit. The official was not authorized to speak publicly.

The developments came a few hours after the White House publicly backed away from Obama's threat to veto any bill that linked the payroll tax cut extension with a Republican demand for a speedy decision on the 1,700-mile Keystone XL oil pipeline proposed from Canada to Texas Gulf Coast refineries.

Obama said on Dec. 7 that "any effort to try to tie Keystone to the payroll tax cut I will reject. So everybody should be on notice."

Obama recently announced he was postponing a decision until after the 2012 elections on the much-studied proposal. Environmentalists oppose the project, but several unions support it, and the legislation puts the president in the uncomfortable position of having to choose between customary political allies.

Republican senators put the price of the two-month package at between $30 billion and $40 billion said the cost would be covered by raising fees on new mortgages backed by Fannie Mae and Freddie Mac.

The fees, drawn from a Treasury Department housing finance market reform plan, would add several thousand dollars to the 30-year cost of home loans guaranteed by mortgage giants Fannie Mae Freddie and Freddie Mac and the Federal Housing Administration.

A worker making a $100,000 salary would reap a tax cut of about $330 through the short-term payroll tax extension.

A version of the fee that circulated overnight would effectively raise the interest rate on a mortgage by one-tenth of one percentage point, but the still-undetermined final version ? awaiting analysis from the Congressional Budget Office ? was expected to be lower.

The measure also provides a 60-day reprieve from a scheduled 27 percent cut in the fees paid to doctors who treat Medicare patients.

Officials said that in private talks, the two sides had hoped to reach agreement on the full one-year extension of the payroll tax cut and unemployment benefits that Obama had made the centerpiece of the jobs program he submitted to Congress last fall.

Those efforts failed when the two sides could not agree on enough offsetting cuts to blunt the measure's impact on the debt.

The failure tees up the issue again for early next year, but it won't get any easier to agree on spending cuts. The

"We'll be back discussing the same issues in a couple of months, but from our point of view, we think the keystone pipeline is a very important job-creating measure in the private sector that doesn't cost the government a penny," said Sen. Mitch McConnell of Kentucky, the Republican leader.

Neither House Speaker John Boehner, R-Ohio, nor his aides participated in the negotiations, although McConnell said he was optimistic about the measure's chances for final approval. The payroll tax cut is unpopular in GOP ranks and another vote in two month could present a headache for GOP leaders.

The State Department, in an analysis released this summer, said the pipeline project would create up to 6,000 jobs during construction, while developer TransCanada put the total at 20,000 in direct employment.

The 1,700-mile pipeline would carry oil from western Canada to Texas Gulf Coast refineries, passing through Montana, South Dakota, Nebraska, Kansas and Oklahoma.

The spending bill would lock in cuts that conservative Republicans won from the White House and Democrats earlier in the year.

Republicans also won their fight to block new federal regulations for light bulb energy efficiency, coal dust in mines and clean water permits for construction of timber roads.

The White House turned back GOP attempts to block limits on greenhouse gases, mountaintop removal mining and hazardous emissions from utility plants, industrial boilers and cement kilns.

Nuclear Regulatory Commission Chairman Chairman Gregory Jaczko (right) testifies with fellow commissioners, from left to right: William Magwood IV, Kristine Svinicki and William Ostendorff before the House Oversight and Government Reform Committee on Wednesday.

The government organization charged with keeping nuclear power safe is having a meltdown. The Nuclear Regulatory Commission consists of five commissioners who direct the work of hundreds of nuclear engineers and other experts. They write the rules for how nuclear reactors operate.

Now four of those commissioners say the chairman of the NRC is a bully who's destroying their ability to do their job.

The feud at the NRC is long-standing, but it boiled over last week when a letter the four commissioners wrote to the White House was made public. In no uncertain terms, they said they simply can't stand NRC Chairman Gregory Jaczko.

At a meeting of the House Oversight and Government Reform Committee on Wednesday, all five sat down together in a House hearing room and listed their grievances:

"The chairman's continued outbursts of abusive rage directed at subordinates within the agency staff ? all members of the commission, including me, have been on the receiving end of this conduct," said Kristine Svinicki, one of the NRC commissioners.

"I'm most concerned that the chairman has made a regular practice of interfering with the ability of the commission to obtain information from the NRC staff," said Commissioner William Magwood. He also said that Jaczko had once berated three female staff members to the point of tears.

And Commissioner William Ostendorff had this to say about Jaczko: "It's about bullying and intimidating behavior towards NRC career staff that should not and cannot be tolerated."

While they stopped short of calling for Jaczko's resignation, the commissioners said they feared for NRC's ability to function.

Congressional members of the committee were more blunt. Republican Jason Chaffetz of Utah read the charges the commissioners made in their letter to the White House and demanded answers from Jaczko:

Chaffetz: "True or false: Ignored the will of the majority of the commission contrary to the statutory functions of the commission."
Jaczko: "I have never ignored the will of the majority in an area that is a commission policy."
Chaffetz: "I'll take that as a false. True of false: Interacted with us, his fellow commissioners, with such intemperance and disrespect that the commission no longer functions as effectively as it should."
Jaczko: "Well, I'm a very passionate person about safety."

Jaczko's response wasn't enough for Chaffetz.

"We've got people who are suffering under this gentleman right here," Chaffetz said. "He is not living up to the duties. I don't believe you. I think the safety and security of this nation is too important. I think you should resign."

However, when the commissioners were asked whether, in fact, the turmoil at NRC had compromised safety at power plants, they said no.

And committee member John Tierney, a Massachusetts Democrat, noted that the four commissioners might share some blame. Jaczko's supporters in Congress ? and he has many, having worked there before he went to the NRC ? have accused the other commissioners of dragging their feet in releasing NRC's report on the Fukushima accident and what it meant for U.S. reactors:

Jaczko: "I think we have had some challenges with ..."
Tierney: "Did you feel that there was an attempt to slow down the release of that report on Fukushima?"
Jaczko: "There definitely was an attempt to prevent the release of the report."
Tierney: "It seems we've got a problem with everybody here."

Jaczko denied the accusations, except to say that he can be outspoken about safety. But advocates of nuclear power have targeted Jaczko because he helped deep-six the idea of putting a nuclear waste dump in Nevada, called Yucca Mountain.

A view from the outside of the Washington hot-house comes from Peter Bradford, who spent five years as an NRC commissioner. He says this isn't just about Republicans versus Democrats.

"There is a sort of nuclear party that transcends Republican and Democratic labels," Bradford says. "The four signers of the letter are all very much in the nuclear party, and the chairman is not."

When asked at Wednesday's hearing if they could patch this up, all four commissioners said maybe. And in fact, according to the NRC website, the commission is described as a "collegial body" that regulates nuclear safety. O;n Thursday, a Senate Committee will query the witnesses again.
